Structure and molecular target

Tesamorelin is a 44-residue analog of growth hormone releasing hormone bearing a trans-3-hexenoyl group on the N-terminal tyrosine. It is structurally distinct from sermorelin and the CJC-1295 series in a way that matters: those are analogs of the 1-29 fragment, whereas this is the full-length 44-residue sequence.

The N-terminal acyl group serves the same purpose the D-alanine substitution serves in modified GRF(1-29): it blocks dipeptidyl peptidase-4 from cleaving the exposed N-terminus, which is the fastest degradation route for any GHRH sequence. The two compounds solve the same problem by different chemistry, one by acylation and one by stereochemical substitution. Its target is the GHRH receptor, a class B G-protein-coupled receptor.

Findings in the published research literature

In-vitro characterisation uses cAMP accumulation assays in cells expressing the GHRH receptor, with pituitary cell preparations for direct examination of releasing activity. Comparative stability work against the unmodified sequence is a substantial part of the reported literature, since resistance to N-terminal cleavage rather than altered receptor affinity is the design objective.

The full-length backbone is the other point of comparison. Residues beyond position 29 are not required for receptor activation, which is why the shorter fragment analogs are active at all, but the reported work examines whether the additional sequence affects receptor residence and conformational stability in ways the truncated analogs do not.

Analytical verification

At 44 residues this is a long solid-phase synthesis and deletion sequences are correspondingly the dominant impurity class. On a molecule this size a single missing residue is a small relative mass shift that chromatography resolves more reliably than a routine mass check, so reversed-phase HPLC purity carries more weight here than on shorter sequences.

The N-terminal acylation requires separate confirmation, since an unacylated lot is the native sequence rather than this compound and carries the dipeptidyl peptidase-4 liability the acyl group exists to remove. The sequence retains asparagine and methionine residues that the modified GRF analogs substituted away, so deamidation and oxidation products are both worth reviewing on incoming documentation.
