Research context

A pre-formulated blend of a full-length GHRH analog and a ghrelin receptor agonist. Supplied lyophilised, with the component ratio verified chromatographically per lot.

Composition and molecular targets

This is a co-lyophilised blend of tesamorelin and ipamorelin, pairing a releasing hormone analog with a secretagogue in the same way the CJC-based blend does, but with a structurally different GHRH component.

Tesamorelin is the full-length 44-residue GHRH sequence carrying an N-terminal trans-3-hexenoyl group, acting at the GHRH receptor through Gs and cAMP. Ipamorelin is a pentapeptide of largely unnatural and D-configured residues acting at the growth hormone secretagogue receptor 1a through Gq and calcium. The difference from the CJC-based blend lies entirely in the GHRH component: a full-length acylated sequence rather than a 1-29 fragment with stereochemical and side-chain substitutions.

Findings in the published research literature

The rationale for combining these two receptor pathways is the co-stimulation effect reported in pituitary cell preparations, where engaging the releasing hormone receptor and the secretagogue receptor together produces a greater response than either alone. That observation predates both of these specific analogs.

As with any blend, published work used the components separately at independently chosen concentrations, so the fixed ratio in a co-lyophilised preparation is a formulation decision rather than a finding carried over from the underlying research. Controlled comparison of this specific pairing against its components is absent from the published record.

Analytical verification

The two components separate readily on reversed-phase HPLC given the substantial difference between a 44-residue acylated sequence and a pentapeptide, and integration of the two peaks establishes the ratio reported on the Certificate of Analysis. A single combined purity figure cannot distinguish correctly proportioned material from a preparation short on one component.

Each component carries its own attributes. Tesamorelin requires confirmation of N-terminal acylation, without which the lot is the native sequence rather than the analog, and its length makes deletion sequences the dominant impurity class. Ipamorelin requires confirmation of C-terminal amidation and of its D-configured residues, both mass-identical failure modes resolvable only by chromatography.
